Diabetes Care Programme

Our multidisciplinary Diabetes Care Team offers comprehensive treatment for diabetic patients. Over 3 to 4 months,
you’ll meet with our specialists, join support groups, and participate in diabetes classes to learn how to manage your
diabetes and potential complications.

If you’re experiencing any symptoms of diabetes, it’s best to get tested at our hospital. Our comprehensive Diabetes Care Programme offers two types of tests to measure your blood glucose levels. 

  • HbA1c test to measure your average blood glucose levels for the past 3 to 4 months. This Hb1Ac test is administered at the start and at the end of the programme to measure the progress you’ve made throughout the programme. 
  • Plasma glucose test will also be administered during each visit to measure your glucose levels on that day. 

We will review your results together and discuss next steps.

Our Diabetes Care Programme aims to create a comprehensive long-term care plan for each patient. Therefore, our patients are scheduled to meet with various specialists to get a better picture of their health conditions and create a holistic and realistic care plan.

You’ll meet with:

  • Diabetes counsellors to learn more about your health condition
  • Dietitians to assess your eating habits and create a diet plan that suits your lifestyle
  • Pharmacists to learn about diabetes medication and assess its impact alongside other medications you might be taking
  • Psychologists or psychiatrists who can help you manage the stresses and adapt to your new lifestyle
What you’ll learn

Apart from personalised consultation with medical experts, our Diabetes Programme includes four educational sessions with trained diabetes educators who will teach you to manage your diabetes in the long-term. Our diabetes education programme is based on the modules outlined by our dedicated multidisciplinary team.

  • Diabetes medication and insulin
  • Signs and symptoms of hypo or hyperglycemia

  • Cardiovascular education including lipids and blood pressure
  • Strategies to control blood glucose levels
  • Side effects of diabetes medication when taken with other prescribed medication
  • hypo or hyperglycemia

  • Strategies to manage diabetes through exercise and nutrition
  • Discover the positive impacts of quitting smoking (if the patient smokes)

  • Deep dive into potential macro and micro complications from diabetes
  • How to detect, prevent or treat complications
  • Education on foot care
